Position will provide a wide range of case management services for HIV positive individuals. The PrEP/nPEP Case Manager will be responsible to educate patients on PrEp/nPEP. Emphasis is on prevention of risk behaviors associated with spread of HIV. Provides group and individual facilitation on risk reduction. Will promote available services for the client within the agency and will refer patients for services outside the agency. Will maintain a flexible schedule in order to facilitate patient care. |
