Position will provide a wide range of prevention services for HIV positive individuals . Emphasis on prevention of risk behaviors associated with spread of HIV. Provides group and individual facilitation on risk reduction. Provide HIV testing and will promote available services for the client within the organization and will refer clients for services outside the agency. Will maintain a flexible schedule in order to facilitate client care. POSITION DUTIES INCLUDE BUT ARE NOT LIMITED TO: Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Prevention Specialist will provide initial comprehensive assessment of the clients' risk behaviors and client needs and personal support systems; development of an individualized prevention plan. Clients receiving HIV testing must be provided risk reduction and post test counseling with referral to ancillary services provided if needed. Assists individuals who test positive for HIV in accessing services. Position requires familiarity with community resources in order to conduct referrals to services including; substance use treatment, STD/TB Clinics, and other services. Provide HIV/AIDS/HCV education to public in traditional and non-traditional settings. Assists senior staff in developing new testing sites and collaborations with other entities. Work closely with medical case managers and other staff to motivate clients toward positive, healthy behaviors and to continue to access other services available for them Prepares monthly reports in a timely manner as requested by supervisor. Prepares time reports and mileage accurately and on time. Serves as an advocate for the clients Ensures that all services given to clients are based on client eligibility guidelines. Must be familiar with performance measures, objectives and grant requirements. Makes referrals to other social service agencies in the area. Provides HIV testing and education Complies with all HIPAA requirements Conducts self in a manner consistent with professional standards Other duties as assigned |
